  <body>&lt;h1&gt;Sage Registration&lt;/h1&gt;  &lt;div id="registration_button"&gt; &lt;a class="register_button" href="/registrations/new"&gt;&lt;span&gt;Register Now&lt;/span&gt;&lt;/a&gt; &lt;/div&gt;  &lt;p&gt;We appreciate your interest in SAGE and look forward to providing you valuable education and training in the field of aging.&lt;/p&gt;  &lt;p&gt;&lt;strong&gt;Please read the following information before registering:&lt;/strong&gt;&lt;/p&gt;  &lt;ul&gt; &lt;li&gt;The Registration Form is designed to provide us with necessary registration information and &amp;quot;holds your place&amp;quot; in the next SAGE Series (or the next SAGE course).&lt;br /&gt; &lt;/li&gt;  &lt;li&gt;The SAGE Series begins Spring Quarter (late-March).&lt;br /&gt; &lt;/li&gt;  &lt;li&gt;You may pay your tuition (on-line or by check) at the time you submit your registration, or pay your tuition by the due date for the specific quarter. &lt;/li&gt;  &lt;li&gt;&lt;strong&gt;All SAGE students are awarded a Certificate of Completion in Gerontology&lt;/strong&gt; from The Ohio State University Office of Geriatrics and Gerontology after successful completion of the SAGE series.&lt;/li&gt;&lt;li&gt;&lt;strong&gt;&lt;em&gt;Current OSU students interested in pursuing SAGE should complete the SAGE registration process and &lt;u&gt;ALSO&lt;/u&gt; contact Linda Mauger at mauger.10@osu.edu or 614-293-8031 for details regarding registration and academic credit.&amp;nbsp; They SHOULD NOT pay tuition fee from this web site.&lt;/em&gt;&lt;/strong&gt;&lt;/li&gt;  &lt;/ul&gt;  &lt;div class="simple_box sm_fltbox"&gt;Each SAGE course is approved for five (5) hours of continuing education credit by The State of Ohio Board of Examiners of Nursing Home Administrators (BENHA), in accordance with BENHA regulations regarding maximum annual online continuing education credit. The BENHA approval number is 11/2008-5.&lt;/div&gt; &lt;p style="margin-bottom: 0pt"&gt;&lt;strong&gt;Continuing Education Requirements&lt;/strong&gt;&lt;/p&gt;  &lt;p&gt;Students are required to complete all assigned readings, review all web resources, view assigned videos and participate in weekly discussions with interdisciplinary class members and faculty throughout the 10 week quarter for each course. Contributions to the discussions are expected to be informed, thoughtful and respectful of other opinions and positions. All students are expected to successfully complete the end-of-quarter written assignment or test in order to effectively demonstrate their proficiency in each SAGE course.&amp;nbsp; Course faculty will determine a &amp;quot;satisfactory&amp;quot; or &amp;quot;unsatisfactory&amp;quot; grade at the end of each course in the series. Students will be granted five hours of continuing education credit (or 15 hours for the series) from the Office of Geriatrics and Gerontology of The Ohio State University Health Sciences Center, which may be presented to professional credentialing organizations for possible continuing education credit.&lt;/p&gt;  &lt;p&gt;&lt;strong&gt;Tuition: $475 per course, or $1,425 for the series (You may pay tuition for each course seperately or the entire series by the tuition due date).&lt;/strong&gt;&lt;/p&gt;  &lt;p align="left"&gt;&lt;br /&gt; &lt;/p&gt;</body>
